New Patients

Whether you're visiting Jeremy Archibald Family & Cosmetic Dentistry to find a new dental care provider or need ongoing oral health and maintenance, we’re very happy to assist you and would like to thank you for choosing us.

What about your smile would you like to change?

Your initial visit helps us understand what priorities and objectives you have for your oral health so that we can best meet your individual dental care needs.

Your first visit will start with gathering important information with one of our assistants and by completing the forms provided by our administrative team. Any necessary X-rays will be taken to assist us in assessing the current state of your oral health. Next, Dr. Archibald will conduct a thorough head and neck exam, oral cancer screening, and a comprehensive evaluation of your teeth and gums. After understanding your individual oral health goals, Dr. Archibald will recommend any treatments necessary to achieve your desired results.

New patients should plan on being at our office for roughly 60 to 90 minutes for this comprehensive assessment. If your schedule allows, we can often coordinate a cleaning with our hygienist on the same day.

If you are experiencing dental or oral pain, please let us know.  We can focus our first exam on addressing this concern and later perform a comprehensive assessment of your oral health.

Steps to take prior to your first dental appointment with Dr. Archibald!

We look forward to meeting you and discussing your dental/oral health care needs and goals. We prioritize being punctual with your appointment.

Prior to your appointment, we encourage you to fill out the patient forms below so we can accelerate your check-in process. Please download, print, and complete the forms and bring them with you to your appointment. Printing and completing these forms in advance of your arrival will help to ensure your appointment starts on time and stays on schedule.

If you are unable to download our patient forms in advance, we recommend arriving at your appointment 15-30 minutes early so that you can have ample time to fill out the appropriate forms. Completing these forms is an important part of having the information needed to fully assess the current state of your oral health.
